The TransplantFirst Academy’s "Champion Patient Program" was developed to inspire Chronic kidney disease (CKD) patients to become their own best advocate by proactively learning more and doing more long before they get ill. This progressively-stages program can be offered within the Nephrology office setting, presented by staff members who can provide an overview and assign home study from written patient modules. Growing evidence shows that patient engagement of this kind leads to better patient outcomes.
