Cognitive decline often goes undetected until it reaches advanced stages, creating a need for accessible and intelligent tools to support early diagnosis and continuous care. Existing systems rely heavily on in-person evaluations, limited specialist availability, and paper based assessments, making the diagnostic process slow and inconsistent. Traditional approaches also lack real-time monitoring and personalized engagement, leading to delays in intervention. CereBloom addresses challenges such as delayed diagnoses, limited access to specialists, and the absence of integrated cognitive health solutions. It aims to provide timely assessments, enable proactive monitoring, and foster collaboration between patients and healthcare professionals. There is currently no unified platform that combines AI-driven cognitive evaluations, interactive exercises, telemedicine, and predictive analytics for comprehensive cognitive health management. The project’s objectives are to create a mobile platform that facilitates early detection of cognitive decline, generate personalized cognitive wellness tracker, and offer engaging exercises that adapt to user performance. It also seeks to streamline patient specialist interactions through telemedicine and provide real-time data insights for better treatment decisions. Ultimately, CereBloom aims to empower individuals and healthcare providers with accurate, timely, and data backed cognitive care. By integrating these features, the project enhances early intervention, improves cognitive health outcomes, and contributes to more efficient, accessible, and continuous care.
