In this sequel starring Shawnee Smith from Saw, the ghosts of Kayako and Toshio return to sow horror as the Grudge saga continues. Even though Jake (Matthew Knight) made it through the terrifying event, the sheer dread of it has almost driven him nuts. While in the hospital, Jake experiences visions of his spectral assailants, and his caregiver, Dr. Sullivan (Smith), embarks on an investigation into his amazing stories. While investigating Jake's Chicago residence, Dr. Sullivan learns that Kayako and Toshio are pursuing a new family. If Dr. Sullivan is unable to persuade the enigmatic Japanese woman to assist in driving the angry ghosts away, everyone concerned could face a fate worse than death.
