Description

The scene of what has been called a systemic failure. The U.S. consulate in Benghazi Libya comes under attack.

Four people including Ambassador Christopher Stevens were killed in the assault.

Now a scathing government review of the attack finds that it did not have to be that way.

(SOUNDBITE) (English) ADMIRAL MIKE MULLEN, THE RETIRED CHAIRMAN OF THE U.S. JOINT CHIEFS OF STAFF, SAYING:

"The security posture at the special mission compound was inadequate for the threat environment in Benghazi and in fact grossly inadequate to deal with the attack that took place that night."

The review found that intelligence provided no "specific tactical warning" of the attack and "little understanding of the threat in the eastern Libya.

Three State Department officials resigned after release of the review.
