Description
The PSCI Population State-of-Health Analyzer utilizes EMR, claims and pharmacy data to perform risk stratification of patients by chronic conditions such as diabetes, coronary heart disease, asthma, osteoporosis and others. Using sophisticated but clinically proven algorithms, the module computes state-of-health scores and segments patients into high, medium and low risk pools and color codes them for easy visualization.
