Unless you check the box "always open in multi-user mode" then Quickbooks opens a file in single-user mode every time.
This is by design and does not require troubleshooting.

When you open the company file for the first time it's usually at the very bottom of the file selection window. If you've already opened the company file simply browse to it again from within Quickbooks and you should be able to select that check box.
Maybe you’re new to Quick books, maybe you’re an old pro, but suddenly you’ve realized that when you log in, Quick books decides you should be in “single user” mode, not “multi-user” anymore. This becomes quiet the headache as everyone has to log out, find out who the first user was in the system, and start all over again.
Well our Critical Edge technicians recently solved this mystery. They can tell you how to resolve this issue: (note you must have the full version of Quick books, the hosting version will not work for this fix)
1. Log everyone out
2.. Uninstall and re install Quick books on the server (or host machine) and then open the company file from the same machine, with the “multi-user mode” checked.
3. Once you have successfully logged in you may close the company file.
4. Log onto other workstations.
The file should open in “multi-user mode” mode by default.

Really? By your instructions, I would need to install the QB program as well as the QBDBMgr on the server. Would it not suffice to open QBWUSER.ini on each machine that runs QuickBooks and ensure that they all have the line:
MULTIUSERMODE=Y?
